BOOKING YOUR APPOINTMENT
Although last-minute appointments are sometimes available, we encourage you to book your treatments as far in advance as possible to avoid disappointment. This is especially true as we near our busy season in November and December.

Also available is online booking through our Facebook page at www.facebook.com/revariewellness. Please note our receptionist is part time, so we may not be able to accommodate in-person requests if our therapists are busy assisting other clients.
If you need to change, reschedule or cancel an appointment please notify us as soon as possible. We have reserved this time exclusively for you, and a notice period is required to accommodate those waiting or wishing to reserve with us. Your appointment can be changed or cancelled up to 24 hours prior to your appointment time at no charge.
Changes or cancellations within 24 hours are charged as follows:
Please plan to arrive at least 10-15 minutes prior to your first treatment to allow time to check in and fill out a Health History Form on our tablet, if you have not already filled out the forms online.
To avoid disappointment, please be on time. If you are late, it will result in a shortened appointment or a rescheduled appointment with applicable charges. Arriving late will limit the time of your treatment. Your treatment will end at the originally scheduled completion time so that the next guest will not be delayed.
Should you arrive more than 15 minutes late for your scheduled appointment time, the appointment will be cancelled and will be subject to our cancellation policy.
Acknowledgment of the cancellation and Missed Appointment policy are required during time of booking, through our online intake forms. Making an appointment with us is notice that you agree to the cancellation and missed appointment policy.
Revarie Wellness is bound under the Personal Health Information Privacy Act (PHIPA) and Personal Information Protection and Electronic Documents Act (PIPEDA) as a Health Information Custodian. More information, including our Privacy Statement, can be requested and viewed at the clinic.
Revarie will retain your files in accordance with these acts and regulations of the College of Massage Therapists of Ontario.
We will only release your information when we have your consent; or where we are required or permitted to do so by law. It will not be sold, or rented to any individual, group, corporation or agency.
Clients transferring from another clinic may be asked to sign a release of health information form in order to comply with regulations as set out by PHIPA and PIPEDA.
CREDIT CARD INFORMATION
Due to a large number of missed or late cancellations, we have decided to require credit cards to secure first appointments. We are not able to offer short notice appointments to clients who may be waiting for openings when we do not have sufficient notice and it's not fair to those who are in need of treatment. As RMTs, we are all self employed and as such, missed appointments means missing money on our paychecks.
This policy will take effect December 15, 2020 for all new clients and any returning clients who have abused our cancellation policy in the past.
We want to assure you that we will bill your credit card in the following situations only:
1. There is an outstanding balance on your account, or
2. Your balance is 30 days past due and we have sent you at least 2 statements, or
3. Your insurance card is invalid and you do not have another insurance, or
4. You have missed or cancelled an appointment outside of the 24 hour cancellation window.
Under PHIPA and HIPAA, we are under strict rules and guidelines in terms of protecting patient privacy and the credit card is considered protected health information. Because of HIPAA rules, our medical office is far more secure than most retail establishments as it relates to identity theft. Furthermore, our server does not store your full credit card number, only a "token" which is used to replace sensitive information that is used by our app's payment processing system.
We understand that not everyone has a credit card. If this is the case, you are welcome to leave a deposit in cash or on debit that we may use in the above-mentioned situations. Please contact us to discuss this before booking an appointment.
